About pain
Pain is the most common complaint and reason for everyone to go to a doctor. But, not everyone knows that pain is a protective mechanism or a warning to prevent further tissue injury. Types of pain include Acute Pain, Chronic Pain and Cancer related Pain. Acute Pain is usually of short onset and regularly related with specific injury; lasting from seconds to 6 months Chronic Pain which is continuous or repeated pain that persists beyond the expected curing time and infrequently recognized to a specific cause or injury; lasts for 6 months or longer Cancer related Pain which can be acute or chronic and directly linked with the cancer, a result of chemotherapy or radiation. Classifying pain by location and etiology aids in communication about and treatment of the pain and to predict course of pain and plan effective treatment using this categorization, respectively. Balanced analgesia is a concept of using more than one form of analgesia concurrently to obtain more pain relief with fewer side effects. Two or thr
ee types of agents are used together to maximize pain relief while minimizing the possibly toxic effects of a single agent.
Nonopioid Analgesics (NSAIDs)
These are First class of drugs used for treating pain which treats pain of various etiologies like surgery, trauma, arthritis, and cancer. These have a ceiling effect to analgesia (a state of no pain). Ceiling effect is the state of a dose beyond which no improvement in the analgesic effect is noted and increase in side effects can occur. No tolerance or physical dependence is caused by the use of NSAIDs. Most nonopioids have antipyretic (body temperature reducing) effects and work primarily at the site of injury, or rapidly. NSAIDs block synthesis of prostaglandins which are principal mediators of inflammation and end products of the arachidonic acid pathway which is a cascade of reactions that occur after tissue injury. Examples of NSAIDs are salicylates (aspirin); NSAIDS (ibuprofen, ketorolac, and naproxen); COX-2 inhibitors (celecoxib; generic Celebrex); acetaminophen (paracetamol).
