The Battle Against Tuberculosis : Undertstanding Expert Insights from Dr Kumar Doshi Chest Physician and Pulmonologist in Ghatkopar, Mumbai
IntroductionTuberculosis, commonly referred to as TB, is one of the major health concerns of present times. Though TB dates back centuries, it still infects millions of people worldwide each year, especially in developing countries. Besides the fact that its persistent nature, alongside drug-resistant strains, has posed a serious concern despite improvements in medical science.
Dr. Kumar Doshi is an interventional chest physician, a pulmonologist who has gained experience in the medical field for more than 25 years. He will be sharing his knowledge with us about TB, its symptoms, causes, and effective treatments.
What is Tuberculosis? Tuberculosis is a bacterial infection; it is an airborne disease causatively caused by Mycobacterium tuberculosis. The main organ involved in tuberculosis infection is the lungs, although it may affect other organs of the body, which include the kidneys, spine, and brain. It is transmitted through air droplets during coughing, sneezing, or even talking, hence being a very contagious disease in badly ventilated places with overcrowding.
Symptoms of Tuberculosis The symptoms of active TB depend on the part of the body affected, but generally, pulmonary TB-the most common type-always presents the following: Prolonged cough, usually more than 3 weeks Chest tightness Coughing out blood Fatigue and a general feeling of weakness Unintentional weight loss Nocturnal sweats and fever Loss of appetite If you are experiencing any of the above symptoms, you must seek urgent medical attention. The earlier TB is detected and treated, the less likely it is to spread to other parts of your body.
Treatment of Tuberculosis :The course of treatment for TB is quite intensive and spans over a rather long period, often up to several months. The standard course includes combination antibiotics for a period of 6-9 months. Commonly used drugs in this course include:
Isoniazid (INH) Rifampin (RIF) Ethambutol (EMB)Pyrazinamide (PZA) The symptoms may get better before finishing the course, but the full treatment should be completed. This prevents drug-resistant TB from occurring. It would then be more difficult to treat and possibly more virulent.
Dr. Doshi emphasizes direct observation of therapy or DOT: the provider is responsible for observing the patient take their medication to avoid partial treatment and the risk of drug resistance.
Drug Resistance TB: The Challenge One of the major concerns facing TB treatment today is multi-drug-resistant tuberculosis. This takes place when bacteria become resistant to at least two of the most powerful TB medicines: isoniazid and rifampin.
