Congenital Vertical Talus: Understanding the Condition and Available Treatments

Congenital vertical talus (CVT), also known as congenital convex pes valgus or rocker-bottom foot, is a rare rigid foot deformity present at birth. It occurs in approximately 1 in 10,000 live births and affects boys and girls equally, with bilateral involvement in about half of cases. In this condition, the talus bone is oriented vertically rather than pointing toward the toes. This leads to dorsal dislocation of the navicular bone over the talar head, equinus and valgus of the hindfoot, and dorsiflexion and abduction of the midfoot and forefoot. The result is a stiff, convex sole that resembles the rocker of a rocking chair.
The exact cause remains unclear. More than half of cases are idiopathic, occurring in otherwise healthy infants. The remainder are associated with neuromuscular disorders, genetic syndromes, or chromosomal abnormalities such as spina bifida, arthrogryposis, myelomeningocele, Marfan syndrome, or trisomies. Without treatment, the deformity becomes more rigid over time. Weight-bearing leads to painful calluses over the prominent talar head, difficulty fitting shoes, abnormal gait, and long-term disability. Early diagnosis and intervention are therefore essential.
Diagnosis begins with clinical examination. The foot shows a rigid rocker-bottom appearance that does not correct with passive manipulation. Deep creases appear on the dorsolateral aspect, and the talar head is palpable medially and plantarly. Radiographs confirm the diagnosis: lateral views in forced plantar flexion demonstrate persistent dorsal dislocation of the talonavicular joint and a vertical talus that fails to align with the first metatarsal. These imaging findings distinguish CVT from more flexible conditions such as calcaneovalgus foot or oblique talus.
Treatment aims to achieve a plantigrade, flexible, pain-free foot capable of normal weight-bearing and shoe wear. Modern management prioritizes early, minimally invasive approaches over the historically extensive soft-tissue releases that carried higher risks of stiffness, avascular necrosis of the talus, and wound complications.
Non-surgical measures form the foundation of care and begin as soon as the diagnosis is confirmed, ideally in the first few months of life. Serial manipulation and casting using a reverse Ponseti technique stretch the contracted dorsal and lateral soft tissues while gradually reducing the talonavicular joint. Weekly casts (typically four to seven) progressively plantar-flex and adduct the midfoot and forefoot. Stretching exercises performed by caregivers complement casting. Although casting alone rarely achieves full correction because of the tight Achilles tendon and irreducible dislocation, it significantly improves foot flexibility and reduces the extent of subsequent surgery.
Most children require a limited surgical procedure after casting, usually performed between 6 and 12 months of age. The contemporary gold-standard technique is the Dobbs method. It combines percutaneous Achilles tenotomy (to correct equinus) with a small medial incision for open reduction of the talonavicular joint, followed by temporary pinning of the joint with a Kirschner wire. The wire is left in place for approximately six weeks while a long-leg cast maintains the corrected position. This approach minimizes soft-tissue dissection, preserves joint mobility, leaves smaller scars, and yields excellent short- and intermediate-term radiographic and clinical results. More extensive single-stage or two-stage open releases of multiple tendons and joint capsules are reserved for severe, rigid, or neglected cases, or for older children in whom casting has failed.
Postoperative management is critical to maintain correction. After pin and cast removal, children wear a brace—typically a foot-abduction orthosis with an abduction bar—nearly full-time for three to four months, then at night and during naps for up to two years. Daily stretching of the dorsolateral soft tissues continues. Physical therapy supports strength, flexibility, and gait development. Close radiographic and clinical follow-up detects any recurrence early so that additional casting or minor procedures can be instituted promptly.
Outcomes are generally favorable when treatment begins early and an underlying neuromuscular condition is absent. Most children achieve a functional, plantigrade foot that allows normal play, running, and standard footwear. Residual mild stiffness or radiographic abnormalities may persist but rarely cause significant functional limitation. Prognosis is more guarded when Cvt occurs as part of a broader syndrome; these children often require lifelong multidisciplinary care. Untreated or late-treated deformity leads to progressive pain, callosities, and disability that may necessitate more complex reconstructive procedures, including arthrodesis, in adolescence or adulthood.
Congenital vertical talus is a rigid deformity that demands prompt, specialized intervention. The current standard of care—serial reverse-Ponseti casting followed by minimally invasive reduction and pinning, then prolonged bracing—offers reliable correction with low morbidity. Early referral to a pediatric orthopedic specialist experienced in foot deformities optimizes the chance of a pain-free, functional foot and a normal quality of life.
